The treatment, in plain terms
It's quieter than most people expect. You sit upright in a comfortable chair, fully awake, with a small device positioned near your head. No anesthesia, no hospital gown, no recovery room.
The coil is positioned over the left dorsolateral prefrontal cortex. Most patients read, listen to music, or rest.
The pulses pass through the scalp and stimulate underactive neurons in cortico-limbic networks implicated in depression. You'll feel a tapping sensation. No medication, no anesthesia.
Five days a week, for six weeks. You drive yourself to and from the appointment. Most return to work the same day.
Patients who respond typically notice changes between weeks 3 and 6 — better sleep, restored interest, a quieter inner voice.
What we treat
Major depressive disorder that hasn't responded to at least two adequate antidepressant trials. The primary FDA-cleared indication for TMS.
For patients with OCD who haven't responded to SSRIs or exposure therapy. Different stimulation protocol than depression.
Comorbid anxiety alongside major depression. Often improves alongside the primary depression treatment.
For patients ages 15–21 with major depression who haven't responded to medication. Treatment protocols adjusted for younger patients.

The treatment room
Our treatment space was designed to look as little like a hospital as possible. Soft daylight, a reclining chair, a folded blanket if you want one. You're fully awake and in control the entire time.
Your first visit
Most patients begin TMS within two to three weeks of their first call.
Our intake coordinator gathers basic information — what treatments you've tried, your insurance, your schedule. Twenty minutes, no commitment.
We confirm coverage with your insurer. For most plans, treatment-resistant depression is a covered indication for TMS. We'll tell you what your out-of-pocket cost will be before you commit.
You meet with Dr. Shamasundara for a one-hour evaluation. Medical history, treatment history, current symptoms. She tells you honestly whether TMS is appropriate.
Your first session begins with motor-threshold mapping — calibrating the coil dose to your brain. About 30 minutes, once.
Five days a week, roughly 20 minutes per session. Most patients schedule before work or during a lunch break.
